Community Care Program

  • How do I apply for services?

    In order to determine eligibility, we will have to complete an intake form to set up a home visit.  After the intake is completed, a Care Coordinator will come out to your home to complete a comprehensive evaluation to determine your eligibility for services.

  • What will be required during the comprehensive evaluation?

    During the home visit, we will need you to provide us with:

    • Proof of age (State ID or Driver’s License)
    • Proof of all bank accounts (savings, checking)
    • Proof of assets (life insurance policies, retirement accounts such as 401k)
    • Proof of income (Social Security Award letter, Pension Statement)
    • Proof of social security number (social security card, possibly your Medicare card)
    • Proof of Medicare and any other health insurance
    • Apply for Medicaid (a government medical program)
    • Provide a list of all medications taken

    If you are applying for Home Delivered Meals or Chore Housekeeping only, then financial documents and applying for Medicaid are not required. For all other services, these documents must be provided for you to be approved for services.

Caregiver

  • Am I a Caregiver?

    You are a caregiver if you are the person in charge of your loved one’s care. Although you might not be getting paid for taking care of your loved one you have taken on the responsibility of helping with tasks such as 

    • Providing transportation
    • Preparing meals and housework
    • Assisting with personal hygiene
    • Organizing medication
    • Providing emotional support
    • Handling finances
    • Other unpaid tasks
  • Can I be my loved one’s paid Caregiver?

    Yes, you can be your loved one’s paid Caregiver once your loved one is deemed eligible for the Community Care Program. The Community Care Program has eligibility requirements that your loved one will have to meet. To apply your loved one for the Community Care Program you will need to contact our Intake Department. housework, medication, finances, emotional support, and transportation.

  • What is the Caregiver Resource Center’s service area?

  • What is Respite?

    Respite services provide a paid caregiver to care for your loved one at home while you take a break from caregivi

  • Can Respite services be provided overnight?

    No, respite services can only be provided during the day; the latest being 10 pm.
