Community Care Program

Community Care Program

The Illinois Department on Aging’s subsidized in-home care program provides a variety of services for older adults who need assistance with activities of daily living on a regular basis. Eligibility for this program is based on the need for care and financial criteria. A Care Coordinator can provide an in-home assessment to determine eligibility for this program. Services provided by the Community Care program include: homemakers services, adult day care, automated medication dispenser, and emergency home response systems.

Comprehensive Care Coordination

Solutions for Care employs professionally-trained staff who can assess the needs of older adults facing a change in health or functional ability on an individual basis. Care coordinators can offer information, assistance and referral to in-home services designed to address the needs identified.

Care Coordinators can conduct consultations in a variety of settings; the home, local rehab facilities, and the hospital. Assessments are based on the unique needs of each older adult and existing resources. With the older adult and/or family members the care coordinator will develop a plan of care that utilizes public and private resources, family support, and the older adults preferences.

Care Coordinators will assist with the implementation of the Plan of Care by making referrals and linkage to services. The Plan of Care can include the following:

  • In-Home Services
  • Adult Day Services
  • Emergency Home Response
  • Automates medication Dispenser
  • Bi-Weekly Chore Housekeeping
  • Home Delivered Meals

In Home Services

In-Home Service provides assistance with household tasks such as cleaning, planning and preparing meals, doing laundry, shopping and running errands. Home Care Aides also assist participants with personal care tasks, such as dressing, bathing, grooming and following special diets.

Adult Day Service

Adult Day Service Services offered in adult day service centers include health, medication supervision, personal care and recreational/therapeutic activities. Nutritious lunches and snacks are served, and special diets are provided.

Emergency Home Response

Emergency Home Response Service (EHRS) is a 24-hour emergency communication link to assistance outside the home for older adults with documented health and safety needs and mobility limitations. This service is provided by a two-way voice communication system consisting of a base unit and an activation device worn by the client that will automatically link the older adult to a professionally staffed support center.

Chore Housekeeping

Provide assistance with house household tasks and daily activities.

Home Delivered Meals

Meals delivered to a client’s home. Individual aged 60 and over are eligible for a home delivered meal if they are determined to be frail or homebound because of illness, disability or isolated. An assessment is required to determine eligibility.

Choices for Care

Aligned under the Community Care Program is a sub-program known as Choices for Care. Hospital Care Coordinators from the Care Coordination Units conduct assessments in hospitals prior to patient discharge. The assessment process gives older adults and their families the opportunity to ask questions, discuss their needs for services, and understand community-based, as well as institutional options for continuing care. In instances of short-term nursing facility placements for purposes of rehabilitation or convalescence, Care Coordinators may conduct follow-up screenings in the nursing facility to facilitate reentry into the community when possible. Solutions for Care provides Hospital Care Coordinators in MacNeal Hospital in Berwyn, Loyola University Medical Center in Maywood, Hines Veterans Hospital in Hines and RiverEdge, Madden Mental Health Center in Forest Park as well as local rehab facilities within our service area.
