Advocate – Aging & Disability Resource Network (ADRN):
The ADRN Advocate will link consumers to available community-based services and appropriate benefit programs such as LIHEAP, SHIP counseling, Benefit Access, Medicare and Medicaid programs, and Options Counseling (if applicable).
This position will also create public awareness through educating the community with presentations and participation in health fairs and community events.
Care Coordinator – Care Coordination Program:
The Care Coordinator is responsible for managing a caseload and conducting in-home assessments in the Berwyn, Cicero, and Proviso Township service areas. Utilizing a comprehensive assessment tool during client home visits, the Care Coordinator will assess eligibility for programs and services underneath the State of Illinois Community Care Program. Assessments include case planning, problem-solving, benefit linkage, and referrals to other community agencies to assist and improve the daily living activities of clients. Care Coordinators work independently and will be expected to complete and maintain required paperwork and client files according to state timeframes.
