Care Coordinator Team Lead
The Team Lead Care Coordinator is responsible for training Care Coordinators. With the assistance of the Care Coordinator Supervisor, the Team Lead must ensure trainees pass the Illinois Department on Aging (IDOA) Certification Test, and complete files accurately, thoroughly and timely. The Team Lead will possess a leadership role and will assist the Care Coordinator Supervisor in their absence. Under the leadership of the Care Coordinator Supervisor, the Care Coordinator will personify this mission statement. The Care Coordinator is responsible for managing a caseload and conducting in-home assessments in the Berwyn, Cicero, and Proviso Township service area. Utilizing a comprehensive assessment tool during client home visits, the Care Coordinator will assess eligibility for programs and services underneath the State of Illinois Community Care Program. Assessments include case planning, problem-solving, benefit linkage, and referrals to other community agencies to assist and improve in the daily living activities of clients. Part Time Receptionist
Part Time position hours are 12:00 pm to 5:00 pm, 25 hours a week. This position is primarily responsible for providing coverage for receptionist position including answering agency phones, directing all incoming calls appropriately and other office support as requested.
