Care Coordinators conduct an in-home assessment which examines an older adult’s environmental, physical, cognitive and psychological and social well-being to design and implement a plan of care in order to allow the individual to stay in their home as long as possible. The assessment includes identification of existing support systems and the need to coordinate with other professionals in the community (such as meal providers, home health care, housekeeping etc.)

Services offered are Home Delivered Meals, Weekly Non-Medical Homemaker services, Adult Day, Emergency Home Response and Monthly Chore Housekeeping.
