General Statement of Duties:
Solutions for Care (SFC) is committed to preserving the dignity of all older adults and their families. Under the leadership of the Executive Director, the Aging and Disability Advocate (ADA) will personify this mission statement.
This is a bilingual position and is the first point of contact for all clients contacting our office regarding information about programs and services for the elderly, their caregivers and the disabled either by phone or in the office. The ADA will maintain current information on opportunities and services available within the (SFC) service areas which include Berwyn, Cicero, and Proviso Township area. The ADA will link consumers to available community-based services and appropriate benefit programs. This position will also create public awareness through community presentations, health fairs, conducting one on one options counseling using a variety of mediums. This position reports to the ADRN Supervisor.
Bilingual in Spanish is required. Candidate must have Bachelors degree in Social Work, or another related area. LIHEAP, SHIP or SHAP certification preferred. Candidate must be adaptable to changes in workload and volume and display excellent communication skills, both verbal and written. A successful candidate must be detail oriented, organized and possess good time management skills are required. Candidate must be skilled at working with clients in-person and over the phone. Proficiency in Microsoft Word, Microsoft Outlook, Excel and PowerPoint are required for this position. Some travel required to outposts, presentations and training, car and car insurance required.  Criminal background check is required.
Duties and Responsibilities


-Develop excellent knowledge of programs,
-Benefits and services available to seniors and adults with disabilities on a local, state and federal level


-Complete required initial and annual training by state and other funding sources; maintain required certification.


-Display appropriate telephone etiquette during all phone call handling


-Maintain a professional and confidential manner in all interactions with the public


-Assess client needs in a sensitive and professional manner and assist with applications


-Develop short term and long term goals for both older adults and the disabled


-Connect clients to appropriate resources


-Provide follow-up to ensure needs are met


-Assist in completing required reports to funders as needed


-Ensure a high level of quality when documenting information


Conduct scheduled presentations to community partners


-Maintain and organize resources (files, books, or a computerized system) so the information is easily accessible
-Update resource information at least quarterly
-Complete all required forms in a timely manner
